Our Office Manager is happy to help you schedule appointments, complete necessary paperwork, and answer any billing questions that you may have.

Patient Appointments

Patients are seen by appointment only. We make every effort to be on time for our patients, and we ask the same courtesy from your family. If you cannot keep an appointment, please notify us immediately, giving a minimum of 48 hours notice to avoid any missed appointment fees. We realize that unexpected illnesses and emergencies may occur, but we ask for your assistance in this regard.Our office will make every effort to schedule an appointment that will work for you.

If you have insurance we will help you determine the coverage you have available. Our office accepts most PPO insurance plans, however, we do not accept Denti-cal or HMO plans.

In order to best assist you, it is important to submit your dental insurance information with your new patient paperwork at least one day in advance of your scheduled appointment.

This allows us time to contact your insurance company and obtain a breakdown of the benefits you (or your employer) have purchased from the company. Our business team will be happy to help you understand your policy.

In the event you are are diagnosed with treatment necessary to be rendered, we will be able to provide you with a detailed plan which will give you an estimate of how much your insurance company may pay for each procedure, as well as the portion you will be expected to pay at the time of service.

Professional care is provided to you, our patient family, and not to an insurance company. The insurance company is responsible only to the patient as a benefit from the employer or individual plan purchase, and the remaining responsibility lies on behalf of the patient to the office. We will assist you in every way we can in the filing of your dental claim and handling of insurance questions from our office on your behalf.

Appointments & Consultations

New patients are always welcome and accepted and our best referrals most frequently come from our existing patients. We are happy to set up your initial visit to our office. Craig Kubina, DDS is also happy to provide a second opinion consultation if you so desire to review treatment that has been previously diagnosed. It is important to us that our patients have a full understanding of the recommended course of treatment, and we make every effort to provide all available information on the different treatment options for your informed decision at Craig Kubina, DDS, before he provides you with his diagnostic opinion.

All patients that are referred to our office, or have referred themselves from another practice, will need a comprehensive exam in order for an accurate diagnosis to be given. It is often very helpful if you have X-Rays from your previous office transferred to us on or before your initial visit to our office.

It is important that we have all dental insurance information and current medical history at the time of your first appointment. We will be providing you with a comprehensive treatment estimate after your consultation with the doctor.
